The information you provide will be forwarded to our dedicated Customer Service team to update your customer profile, which may take up to five working days. Family members or friends can also use this form to inform us of any assistance we can offer to our customers.

We have a tell us once approach to additional needs. This means the information you provide will be shared with other relevant Quilter business units you work with.

Would you like further information on Power of Attorney (POA)?

Power of Attorney is a way for a person to select someone of their choosing to manage their affairs on their behalf if they are no longer able to. If this is something that could be beneficial, visit gov.uk/power-of-attorney

If you have an existing POA you would like to register with us, please follow the guide on how to do this by sending in the POA document along with this Power of attorney form. Alternatively, you can provide the HMRC code in the free format box and we will apply this for you.

Alternatively, you can add a release of information to the account. This allows the customer to select a person of their choosing to the plan to have information rights only. The selected person would not be able to provide instructions for the account, but they will be able to call or email us on the customers behalf.
